Job Description

Care Co-ordinator required, Southend on Sea, Essex for small care provider (LD).

We are recruiting for a Care Co-ordinator for a small care provider based in Southend on Sea, Essex who is registered with CQC to provide residential care for people with learning disability across four locations all of which are in close proximity to each other.  As a Care Co-ordinator role you will work across all three locations within the provider group.

The post is 40hrs a week, which will include hours office based and hours of active support and shift leading. The post holder will also share in on call responsibilities.

You will need to have:

  • Previous management role/experience
  • NVQ level 3 or 4 in Care. Level 5 or working towards level 5
  • Ability to communicate effectively at all levels
  • Team player
  • Genuine interest in working with people with learning disabilities.

As the Care co-ordinator, you are required to oversee / maintain the care of Service users within the homes and in the absence of your line manager, to take responsibility for the day-to-day running of the homes

Care:

  • Ensuring all Staff Members contribute to the best of their ability to the efficient running of the homes, and to the creation of an atmosphere conductive to the best interests of Service users. Interacting with Service users to ensure that their health care needs are being met and resolving any problems where appropriate.
  • Reporting any ill-health amongst Service users and make requests for GP / Professional visits where necessary. Ensuring meals are of sufficient quantity and good quality, and that Service user’s dietary needs are met.
  • Undertaking general support and personal care as appropriate.
  • Administering prescribed medicines and maintaining the necessary records as per Company guidelines.
  • Auditing Care Plan documentation, to ensure best practice, on a three monthly basis.
  • Auditing of health and safety procedures, finances, medication etc Practicing maximum integrity in all dealings with Service users' personal and financial affairs when required, and avoiding abuse of the privileged relationship that exists with Service users.
  • Working with your line managers to create and update service user care plans, risk assessments and other relevant paperwork.
